TestWorkflowEnvironment does not allow signalWithStart when workflow already exists

SimpleWorkflowTest の再現ケースから始め、TestWorkflowEnvironment、TimeLockingInterceptor、WorkflowStubImpl を通る signalWithStart のパスを追跡します。workflow がすでに存在する場合の signalWithStart のリグレッションテストを追加し、テスト環境が WorkflowExecutionAlreadyStarted なしのスタンドアロンサーバーの動作と一致することを確認します。

Expected Behavior

TestWorkflowEnvironment does not allow signalWithStart when workflow already exists - see exception below.

Actual Behavior

TestWorkflowEnvironment should work the same as standalone server in this matter.
