TestWorkflowEnvironment does not allow signalWithStart when workflow already exists

从 SimpleWorkflowTest 复现器开始，跟踪 signalWithStart 路径经过 TestWorkflowEnvironment、TimeLockingInterceptor 和 WorkflowStubImpl 的过程。在 workflow 已经存在时，为 signalWithStart 添加回归测试，并验证测试环境与不包含 WorkflowExecutionAlreadyStarted 的独立服务器行为一致。

Expected Behavior

TestWorkflowEnvironment does not allow signalWithStart when workflow already exists - see exception below.

Actual Behavior

TestWorkflowEnvironment should work the same as standalone server in this matter.
