Seen while testing #448 with Claude Code doing everything. A task's code step (execute_code) was held for approval by the second-look check, which is right. But the reason shown was "the user asked to analyze an error...", which didn't match what the user asked or what the step did. The hold itself worked as designed.
To look at: the prompt and inputs the second look gets for execute_code steps inside a task run (sentient/agent/second_look*). Does it see the task's request, or something else, such as an earlier error string? Small or alternative models may need the request stated more plainly.
Done when: a task's code step that the second look flags shows a reason that matches the task's request. Check on claude-code/sonnet and qwen3:8b.
