Start with src/main/java/org/phoebus/pv/PVPool.java and trace how createPV and releasePV identify a PV. Reproduce the issue with the provided calls: add my_pv without a prefix, then release loc://my_pv. Done means releasing that prefixed PV finds and releases the resource created without a prefix.

If a PV is added without a prefix, then the release function in the PVPool doesn't find it and does not release it.
