Research direction

Read OpenApp/Engine/src/install/install-orchestrator.ts around HandleServerConfig at lines 2206-2213, the shared path for install and upgrade. Update the behavior so the app schema is added only on first install or an operator opt-out is honored. Verify both install and upgrade paths preserve a removed schema.

Written by the indexing model from the issue text.

Description

OpenApp/Engine/src/install/install-orchestrator.ts:2206-2213 (HandleServerConfig, used by install and upgrade) calls AddExcludeSchema every time. That undoes an operator's deliberate removal; it happened twice in one day on aidp-next (Sales install, Accounting upgrade). Related: #4238, #4547.

Fix
Add the schema only on first install, or honour an opt-out.
