pr: -o/--indent does not pad the header line

Summary
pr -o MARGIN should put MARGIN spaces at the start of every output line, including the page header. GNU does that; uutils only indents the body.

Expected
Match GNU print_header: the left margin is applied to the first blank separator line and to the header text line. Trailer blank lines stay bare (GNU leaves those without the margin too).
