The issue names no file. The starting point is the survival::coxph call with a Surv formula and strata() term, which the package calls to get coefficients. Benchmark that call against a candidate .Call C routine, then check the coefficients match survival bit for bit. The scope of the routine and the API shape are still open questions for maintainers.

From https://github.com/Merck/simtrial/issues/382#issuecomment-6151357774. Speed optimization room might exist for certain calls using functions from {survival}, such as:
survival::coxph(Surv(tte, event) ~ (treatment == "experimental") + strata(stratum), data = d)$coefficients
For example, this could be a direct .Call to a custom, bare metal C routine to cover the exact code paths used by the package while removing the irrelevant logic, while bitwise reproducing {survival} results.
Supporting certain "grammar" (syntax sugar) like survival did (formula interface, etc.) might slow things down for pure speed purposes, but it doesn't mean we should stop trying make good API designs even for bare metal underlying routines.
