調査の方向性
Start with backend/src/middleware/auth.ts and inspect the existing CI test suite for authentication middleware coverage. Add deterministic tests for expired and replayed challenges, including boundary inputs and dependency failures. Done means the tests verify the documented success and failure paths, public behavior, and error contract, and pass in the corresponding CI suite.

説明
Labels: testing, backend, points: 150
File/location: backend/src/middleware/auth.ts
Description:
Challenge expiration and replayed challenge rejection lack regression tests.
Acceptance Criteria:
- Add deterministic tests for the documented success and failure paths, including boundary inputs and dependency failures.
- Run the new tests in the corresponding CI suite and assert the public behavior and error contract.
Notes:
This is a focused regression gap for the cited module; it was not covered by the existing test files reviewed during the sweep.
