Context

  • Framework version: 1.6.769
  • Project: 1409-sprind (shared-tooling mode)
  • Reported: 2026-09-21T14:58:30Z

Description

Symptom

fw audit's CTL-013b check ("review-queue verification re-run") reports a WARN whose
count and named tasks change between consecutive same-session fw audit invocations,
e.g. "0 red" -> "1 of 3 red: T-X" -> "2 of 3 red: T-Y, T-Z", naming a different task
pair each time. Read at face value this looks like stored Verification lines
regressing mid-session.

Root cause

fw verify-queue (called by CTL-013b) deliberately samples a rotating N-task window
(default limit 3, FW_VERIFY_QUEUE_AUDIT_LIMIT) from a larger human-review queue,
persisting a least-recently-checked cursor in
.context/working/.verify-queue-state.json. agents/audit/audit.sh documents this
design explicitly around line 4637-4648. Confirmed via a same-session double-call
diff: fw verify-queue --limit 3 --json run twice, one second apart, selected fully
disjoint task-ID sets.

The WARN text itself, however, states only the current sample's red count and the
names of the red tasks — e.g. "review-queue verification re-run: 2 of 3 task(s) red.
Evidence: Awaiting human review with a failing stored block: T-1013 T-1105." It does
not disclose that this is a rotating slice of a larger queue.

Why this matters

Three independent consuming-project sessions (across three rounds of the same audit
sequence) each spent real diagnostic effort re-deriving the "it's a rotating cursor,
not a regression" explanation from source, because the tool's own output gives no hint
that consecutive runs are sampling different subsets. Cost: one dedicated
investigation task per round (3 total) for a non-defect.

Suggested fix

Add one sentence to the CTL-013b WARN/mitigation text naming the rotating-window
design, e.g.: "Note: this check samples a rotating N-task window of the review queue
each run — a different red-count/task-set across consecutive audit invocations in the
same session reflects the sampling window advancing, not stored Verification
regressing." No functional change to the check itself, only its self-description.

Evidence

  • agents/audit/audit.sh ~4637-4648 (rotating-cursor design comment)
  • .context/working/.verify-queue-state.json (independent per-task ts values)
  • Consuming-project task T-1090 (1409-sprind) — full diagnostic trail across 3 rounds
