Actual result
Portions of certain question prompts are not programmatically associated with their corresponding radio button or checkbox answer sets.
Expected result
All question content and instructional text that applies to an answer group should be programmatically associated with the corresponding form controls so it is announced when users navigate through answer options.
Recommendation
Associate all relevant question text with answer groups using semantic fieldsets and legends, or appropriate ARIA relationships.
Acceptance criteria
Question/instructional text is programmatically associated with its answer group (fieldset/legend or equivalent ARIA).
