What problem does this address?
Default block appender creates duplicate paragraph blocks.

What is your proposed solution?
Match the behavior with the Notes app:
- If the last block is a paragraph, focus it and put the cursor at the end
- If the last block is something else, insert an empty paragraph block and focus on it
