Cloudflare Workers javascript Tests

vite/vite.config.ts sets coverage.enabled and an exclude list, but not all: true. V8 coverage only reports files a test imported, so a source file with no test at all contributes nothing and shows no row. The Cloudflare 95.5% figure is over touched files only.

Files with no matching unit test today, by package: cloudflare 17, deno 21, bun 3, vercel-edge 3, hono 4, effect 2.

Work item. Set coverage.all: true in the base config and record the real baseline before setting any thresholds. Expect the reported numbers to drop.
