Start by reading ADR 0022 and tracing the memory-save CUD decision to see which model role it uses and where the “defaulting to ADD” log is produced. Check the role configuration described in #449 and #448. Run the reported scenario with chat on claude-code/sonnet and the background role on qwen3:8b; done means “I moved to Mumbai” updates the existing Pune memory in a real run.

Seen while testing #471 (#463). When a chat runs on claude-code/sonnet, the memory save's add-or-update decision (CUD) is refused as background work (ADR 0022). The log says "defaulting to ADD". So on that setup, saving a memory never UPDATEs or SKIPs through the model, and duplicates and stale facts pile up.
Fix: run the CUD decision on the fast/background role (which #449 keeps on a non-Claude-Code model unless claude_code_everywhere is on, #448), not on the chat's model. If no model can decide, say so plainly instead of silently adding.
Done when: with chat on claude-code/sonnet and the background role on qwen3:8b, saying "I moved to Mumbai" after "I live in Pune" updates the memory. Check in a real run.
