Problem
skills/reflect/SKILL.md step 2 spawns three reviewers in parallel, and step 3 spawns a synthesizer. Nothing in the prompts tells each reviewer where to write intermediate files. In Claude Code, they share the session scratchpad. During one run, a reviewer wrote digest.txt there, and another reviewer overwrote the same file name while the first was still reading it. The first reviewer then read a file in a different format.
The same risk applies to any skill that fans out parallel subagents (for example, swarm lanes or poteto-mode delegates that write logs such as preview.log).
Suggested change
In skills/reflect/SKILL.md step 2, pass each reviewer a dedicated subdirectory (for example <scratch>/reflect-<lens>/) and tell it to write only there. A one-line note in the poteto-mode Subagents section would cover the general case.
