[Bug]: Sequential thinking output missing

在 macOS ARM64 上启用 MCP sequentialthinking 服务器并指示 Claude 使用它，在 PolyScope 0.13 中复现该问题。没有指定源文件或测试，因此首先定位接收并渲染 sequential-thinking 输出的代码。在其运行期间能够看到 sequential-thinking 的内容即表示完成。

Bug description
When sequential thinking is running, I cannot view the contents of sequential thinking.
Steps to reproduce
- Make sure sequential thinking is installed: https://github.com/modelcontextprotocol/servers/tree/main/src/sequentialthinking
- instruct claude to always use it.
- You wont see the output.
