Find the code that handles ingestion and identify where Gio.FileMonitor can trigger re-queueing when a source changes. Done means changes under sources/ are detected and re-queued while unchanged files remain skipped.

Re-queue changed sources automatically. The ingestion queue is cache-aware (SHA-256), so unchanged files are skipped. See TODO.md > Ingestion.
