Support "Changes" for external vital modules

Description
vitalizer
Currently, "Changes" feature supports only vital.vim, not external modules.
One problem is that vitalizer saves only vital.vim's git hash. We need to consider to refactor vitalizer and save needed data for each external modules (including vital.vim one).
