Define and verify the production behavior when no secret is configured, and document that the memory limiter is per instance and when a shared RateLimiter is needed. Done when startup enforces the agreed secret behavior and the deployment limitation is documented.

Observed
When no secret is configured, the handler generates an ephemeral one, so visitor sessions reset on every restart. A console.warn is printed, but startup continues, including in production. The default rate limiter (memoryRateLimiter) is per-instance, so limits are not shared across replicas.

Suggested fix
Fail at startup in production when no secret is set (or require explicit opt-in), and document that the default limiter is per-instance and a shared RateLimiter is needed for multi-instance deployments.
Found while planning nextlevelbuilder/dewee#1450 (out of scope there).
