Start with the Upload preview version step in preview.yaml and the npm-minor-patch Dependabot group that grouped next and @opennextjs/cloudflare. The issue leaves open whether a preview URL or a smoke check on / should gate merges, so a maintainer decision comes first. Done means Dependabot bumps to the Worker runtime packages are held for human review, and the chosen check is wired into CI.

Defect

Dependabot pull requests cannot read CLOUDFLARE_API_TOKEN, so the Upload preview version step in preview.yaml always fails on them. The build step passes, the PR gets a green-enough signal, and the change merges without anyone ever opening a running copy of it. The next push to main promotes it to 100% of production traffic.

This took kagent.dev down today for roughly six hours.

Evidence

  • #599 bumped next 16.3.8 → 16.4.0 and @opennextjs/cloudflare 1.20.8 → 1.20.10 as part of the npm-minor-patch group.
  • Its preview run (38520354926) built successfully and failed only at Upload preview version. No preview URL was posted.
  • It deployed to production at 15:02 UTC on 2026-10-14. Every route the Worker handles then returned HTTP 500 with Cloudflare error code 1101 (uncaught exception in Worker code). /docs/ kept serving because Cloudflare returns those from the ASSETS binding without invoking the Worker.
  • The build and the deploy both succeeded. The failure was at request time only, which is exactly what a preview would have caught and what CI cannot.
  • #605 reverted the two packages and restored the site.

Note that the declared peer ranges accept this combination: @opennextjs/cloudflare 1.20.10 lists next: ">=15.5.27 <16 || >=16.3.8", and the locked wrangler 4.149.0 satisfies its ^4.148.0. Version constraints would not have blocked this.

Two candidate fixes

  1. Give the preview upload access to the token on dependabot branches, via a pull_request_target job or a separate workflow that uploads the version from a trusted context. This restores a preview URL on every PR, which is the actual missing signal.
  2. Separate the Worker runtime dependencies from the rest. next, @opennextjs/cloudflare, and wrangler are the three that can break production at request time. Pinning them out of the npm-minor-patch auto-group forces a human to open each bump deliberately.

These are not exclusive, and option 2 is the cheaper of the two.

Open question

Is a preview URL enough? A preview catches a crash on /, but only if somebody loads it. A smoke check that requests / against the uploaded preview version and fails the run on a non-200 would catch this class without relying on a reviewer clicking the link. Worth deciding before picking between the two fixes above.
