Start by tracing how the feature form reads and displays Date fields with QGIS Value Maps; the issue does not name source files or tests. Compare that path with the working Integer, Decimal, Text, and Boolean Value Map fields, then reproduce using the trees layer’s inspection_date field. Done when the saved date description appears in preview and edit and its entry is selected.

Description
When a Date field uses a Value Map, the selected value is saved correctly, but after saving the field is shown empty in the app’s feature form.
In QGIS the saved date matches the Value Map entry and its description is shown, so the data is correct and only the display in the app is affected.
Other field types with a Value Map (Integer, Decimal, Text, Boolean) work correctly.

Steps to reproduce

  1. In the app, add a new feature in the trees layer.
  2. Tap inspection_date and select 2026-09-23 – Autumn.
  3. Save the feature.
  4. Open the feature (preview), then tap Edit.
  5. Sync the project and open the attribute table in QGIS.

Actual results
Step 2: ✅ the field shows 2026-09-23 – Autumn.
Step 4: ❌ inspection_date is empty in the form.
Step 5: ✅ the date is saved correctly in QGIS and matches the Value Map (the description is shown).

Expected results
After saving, the app shows the saved value’s description (2026-09-23 – Autumn) in preview and edit, and the entry is marked as selected in the list, the same as QGIS.
