Add hooks for headings, code blocks, blockquotes (including GitHub alerts), and tables while preserving output when hooks are absent. Add a fixture for each hook and run ./build.sh test; meet the stated coverage and CRAP thresholds.

Current State
After the link/image hooks, the other common customization points still use Markdig's fixed HTML.

Tests live in Fanstatic.Test/ (xUnit + NSubstitute); fixture sites in .TestSites/. Run ./build.sh test; new/changed methods need CRAP ≤ 15 and ≥ 75 % line coverage.
Acceptance Criteria:
- Each hook covered by a fixture; no output change without hooks.
