Description

Surface: bin/fw doctor's stale-task-debt check (source repo: bin/fw ~line 1845-1872, block starting # Check: Stale task debt (T-814)).

Defect: The loop reads only last_update: from each .tasks/active/*.md file and compares it to a 7-day cutoff. It never reads horizon: or status:. So a task deliberately parked with horizon: later, or a work-completed task correctly held in active/ pending unchecked Human ACs (a real sovereignty boundary — see T-193 in the consumer project), counts identically to genuinely neglected work.

Measured (consumer project "Pen", 2026-09-21, exact replay of the check's own method): of 96 tasks flagged stale, 75 were horizon: later (deliberately parked backlog) and 11 were status: work-completed with only Human ACs unchecked. Only 10 of 96 (~10%) were genuine neglect. The WARN headline ("96 stale tasks") therefore overstates real debt by roughly 9x in this measurement.

Consequence: re-horizoning a task to later — the doctor's own printed remedy path (fw task update T-XXX --horizon later) — does not remove it from the count, because the check does not read horizon: at all. A human or agent following the tool's own suggested remedy sees no change in the reported number, which reads as the remedy not working.

Suggested fix: skip tasks with horizon: later, and skip status: work-completed tasks (or count them in a separate "awaiting human review" bucket, which the codebase already computes elsewhere for a different check).

Fix surface authority: vendored .agentic-framework/bin/fw, HANDS-OFF in the consumer project (T-1771 there). Reporting upstream per that project's committed remediation route rather than patching locally.
