調査の方向性

説明

When an agent asks questions ("Answer N questions" panel above the composer), the collapsed row shows only the currently selected option (here Other (type your own)) with a 2/4 counter. The question itself is not visible anywhere, so the user can't tell what is being asked. The 2/4 counter also doesn't match "Answer 2 questions", so it's unclear what it counts.

Expected: the question prompt is always shown above or next to the options, and the counter clearly says what it counts (question vs. option).

Environment: T3 Code web UI, Linux (Omarchy/Hyprland), OMP provider.
