Split the Review queue into "Questions to explore" and "Needs review"

Start with make start to inspect the demo project and the Review section, then trace how review items and their detail pane are rendered and how actions are sent to the Assistant. The issue names no files or tests, so locate the relevant UI entry points and use make test for the core tests. Done means the section separates questions from items needing review, questions offer Ask and Dismiss, and the detail pane shows the full item with its source.

Most review items are open questions generated during ingestion, not things that need checking. They are mixed together and the only action is Resolve.
