Problem
src/sequentialthinking/index.ts registers the tool as sequentialthinking, but src/sequentialthinking/README.md calls it sequential_thinking in its tool heading and usage text, so someone looking for that name in a client's tool list won't find it.
Expected
The README uses the registered name. The tool itself is not renamed: a published tool name is part of the server's interface.
