Summary
plugins/evals/skills/plugin-eval/SKILL.md:306 uses ${user_config.grader_run_twice} as the subject of a condition ("unless ... is false") with no inline fallback, so an unexpanded token reads as not false.
Fix
State the token once with the existing fallback form, e.g. "Grader run twice: ${user_config.grader_run_twice} (unexpanded or empty means the default)", refer to "the grader-run-twice setting" afterwards, and remove the plugins/evals/ row from the baseline file. The form is the one plugins/code-tidying/skills/tidy/SKILL.md uses for hard_exclusions. One PR, plus an .changes/evals/ or version-bump entry per the plugin's changelog mode.
Verification
bash plugins/skill-quality/scripts/check-skill.sh plugins/evals/skills prints no Check 31 line and grep -c '^plugins/evals/' plugins/skill-quality/scripts/settings-token-baseline.txt returns 0.
