What happened

In Plan mode (read-only sandbox, approval policy "never") a file write is
denied with:

[sandbox: file access denied under read-only mode]

The tool result then advertises an escalation retry. When the agent retries the
same call with sandbox_permissions: workspace-write and a justification, it
fails with:

Error: the user rejected escalating this operation to "workspace-write"; it
stays denied, so stop and explain instead of working around it

No approval dialog appeared and nothing was shown to me. The message states
that the user rejected the request.

Expected: The operation remains denied, but the error message clearly states
that escalation is unavailable because approval prompts are disabled in Plan
mode. It should not attribute the rejection to the user.

The harness already tells the model that prompts are disabled:

"Approval prompts are disabled in this session: actions that require approval
are rejected automatically - do not request sandbox escalation"

Under the "never" policy, approval requests are automatically rejected.
However, the escalation hint still suggests that an approval prompt will
appear, and the resulting error attributes the rejection to the user.

Relevant files in the bundled Harness:

  • dsh-user-approval/src/index.ts
  • dsh-sandbox/src/escalation.ts

Related: #4120 (configurable permission presets).

This report concerns the error message only. It does not request approval
prompts in Plan mode.

Smallest steps or command that reproduce it
  1. Start a chat in Plan mode.
  2. Ask the agent to create a short text file in the workspace.
  3. The write is denied with the read-only marker.
  4. The agent retries the same call with sandbox_permissions: workspace-write
    and a justification.
  5. The result reads "the user rejected escalating this operation ...",
    although no prompt was shown.
