[Bug]: Plan mode occasionally responds with normal chat text instead of creating an execution plan

The report names no files, tests or entry points, only the macOS desktop app, Plan mode, and trace IDs in the diagnostics block. Start by reproducing with a Plan-mode request and checking whether the mode flag reaches the request that produced the chat reply, using those traces as the reference. Done means Plan mode consistently returns an execution plan, with a test or a logged reproduction showing the mode is respected.

Reproduction steps
Open the Command Code desktop application.
Switch to Plan mode.
Enter a request that requires a plan for execution.
Observe the response.
Expected behavior
When Plan mode is enabled, Command Code should generate an execution plan for the requested task rather than responding as if it were in normal chat mode.
Actual behavior
On two separate occasions, Command Code responded with ordinary conversational text instead of creating an execution plan, even though I had switched to Plan mode.
The issue appears to happen randomly. Most of the time, Plan mode behaves as expected, but occasionally it seems to respond as if Plan mode were not active.
