Línea de trabajo
Read the refusal-fallback and Token Budget continuation paths, including ContinuationEntry, TurnRequestBuilder, and the Continue send options; the issue does not name their files or tests. Trace how the source model’s remaining chain reaches the handover, then check that the Continue send uses that chain and advances to C if B refuses.

Descripción
Follow-up from #7231 (Codex thread PRRT_kwDOPxxmWM6vYWeM).
Problem
A refusal fallback whose request reaches a Token Budget stage hands the turn over to a "Continue" on the fallback model (#7231). That Continue is a new send on the fallback model B, so TurnRequestBuilder resolves B's own configured refusal chain. When the source model A has an ordered chain such as [B, C] and B refuses the Continue, the turn uses B's chain (often empty) instead of advancing to C. With the usual configuration the Continue then fails terminally, although A's chain still had an unused fallback.
Proposed fix
Carry the unused part of A's chain ([C]) across the handover: on the continuation entry and on the Continue's send options, and use it in place of B's resolved chain for that one send. This needs new in-memory state on ContinuationEntry and the send path, which is why #7231 did not include it.
Trigger
Next change to the refusal-fallback or Token Budget continuation path. Owner: the #5286 lane.
