Línea de trabajo
Compare outline and breadcrumb behavior between languageserver 0.3.16 and 0.3.17 using the R script in the issue, then trace the code-section symbols returned to Visual Studio Code. The issue names no source files or tests, so start by locating the outline and symbol handling entry points. Done means subsections appear nested under sections and breadcrumbs work with the current languageserver version.

Descripción
Hello,
In version 0.3.16, the Outline view showed the sections and subsections in an hierarchical way which is the expected behavior according to https://github.com/REditorSupport/vscode-R/wiki/R-Language-Service#code-sections and https://code.visualstudio.com/docs/editing/getting-started/userinterface#_outline-view
Starting with 0.3.17, including the latest 0.3.19, the folding disappeared which makes navigation in huge files painful.
It is somehow related to #685 but not exactly the same issue.
It would be great if this issue could be fixed in order to use this folding feature with the latest languageserver version.
Thanks in advance!
Steps to reproduce
Create an R script with the following content
# First section ----
## Subsection of the first section ----
var <- "dummy"
## Another subsection ----
variable <- 2
# Second section ----
another_var <- "Hey!"
Open the Outline view in Visual Studio Code (could be in the Explorer panel depending on your settings).
Expected behavior
The header of the subsections must be hidden and folded inside the headers of the sections:
Observed behavior
All symbols including headers are on the same level:
Note that the breadcrumbs are also broken.

One may downgrade/upgrade easily languageserver with:
install.packages(
pkg = "https://cran.r-project.org/src/contrib/Archive/languageserver/languageserver_0.3.16.tar.gz",
repos = NULL
)
The problem appears starting with 0.3.17.
