Línea de trabajo
Start by reproducing the recurring PIN notifications with Firefox's native OS client certificate implementation on Windows, then inspect the reported CardAuthenticateEx → CardSignData → CardDeauthenticateEx cycle and the preceding "HANDLES CHANGED" notification. Check whether the behavior also occurs with another long-running process such as Outlook; done means identifying the cause or a clearly scoped fix that stops the notification flood without relying on stopping opensc-notify.exe.

Descripción
Problem Description
Windows Firefox is causing a constant flood of PIN verification notifications when used with its native os client cert implementation (default). The log shows, that the cardmod module is causing these messages, because firefox.exe is cycling through CardAuthenticateEx -> CardSignData -> CardDeauthenticateEx round about every 30 seconds. This cycle starts with a notification "HANDLES CHANGED", which indicate a change on the PC/SC level, that may be related. If CardAuthenticateEx was executed successful with the Minidriver's cached PIN causes OpenSC to send a notification.
The problem may be reproducable also with other long-running processes, e.g. Outlook.
Proposed Resolution
I'm not exactly sure what causes the need for a PIN verification, since the os client cert implementation focuses purely on the NCRYPT_KEY_HANDLE, which transparently handles the hardware token abstraction.
Simple workaround is to stop opensc-notify.exe, which suppresses the notification. Another possible workaround would be to disable notifications in cardmod or specifically in Firefox.
