Problem: Ctrl+Shift+F (Toggle fast mode) does nothing visible when fast mode is unavailable, for example with a non-first-party Anthropic base URL.

Cause: toggleFastMode returns early when getFastModeProvider is null (App.tsx).

Fix idea: show a short toast that says why fast mode is unavailable.
