Observed

Initialization of the per-node hub for OIB/LegalEntity/TEST-OIB-TW-Co (instance memex-cloud, pod memex-portal-deployment-5897c49648-jtqxn) failed with the hub entering FAILED state. The failing chain, read off the stack: MeshWeaver.Data.TypeSource1.Initialize→InstanceCollection constructor (src/MeshWeaver.Data.Contract/InstanceCollection.cs) building an immutable dictionary keyed via the identity function → MeshWeaver.Messaging.Serialization.TypeDefinition.GetKey(Object instance) (src/MeshWeaver.Messaging.Hub/Serialization/TypeDefinition.cs) throwing NullReferenceException. So the hub could not index the instances of some type T during buildup — GetKey` dereferenced null for one of the instances (or the key accessor resolved to nothing).

The five occurrences in this incident are one event, five watchers: the CompileWatcher log site reported the same hub-init failure through five of its watcher names (Compile watcher, ReleaseRequestWatcher, [LateReleaseAdoption], Sources watcher, Adopted source-stamp watcher), each saying it is re-establishing. Two sibling fingerprints fired at the same instant with the identical stack on the same pod and node — see Admin/_LogIncident/e740675ef2d0666e (MessageHub site) and Admin/_LogIncident/392512339f030b64 (DataContext site). They are the same event at other log sites, not separate defects.

Where and when

A single burst, ~21 ms wide, on one pod of the tenant instance memex-cloud — no steady rate, no other pods. It lands inside a window of active OIB workspace churn on that instance: an OIB/Litigation NodeType compile failure ~17 s earlier (which itself lists OIB/LegalEntity/Source/* nodes among its discovered sources), and StuckRoll / silo-unavailable alarms on memex-cloud earlier the same evening. The burst therefore coincides with an OIB source/release/sync window rather than standing alone.

Impact

The node OIB/LegalEntity/TEST-OIB-TW-Co — whose id suggests test data — did not serve while its hub was FAILED, on the one pod that activated it. All five watchers reported re-establishing. Whether the node recovered is not established.

Not established

  • The cause. The evidence fixes the failing call and the operation, not why GetKey saw a null.
  • Whether the hub recovered after the watchers re-established, or fails on every activation since.
  • Whether other nodes / pods show the same: this is the only pod and only node path in the burst.
  • The node's own content: the OIB tenant data lives on the memex-cloud instance and is not readable from this control instance, so the instances fed to the InstanceCollection could not be inspected from here.
  • Which type T the faulting TypeSource was initializing — the stack does not carry it.

Candidate causes and the reading that separates them

  1. A stored instance under the node's subtree whose key member is null. GetKey computing the identity of each instance hits a null key. Predicts: the hub fails on every activation, on every pod of that instance, until the data is corrected. Reading: read the node content / typed instances under OIB/LegalEntity/TEST-OIB-TW-Co on memex-cloud and look for an instance whose key field (or the object carrying it) is null.
  2. A mid-adoption type-definition mismatch. Instances deserialized against a changed CLR shape while a NodeType release was being adopted — consistent with the [LateReleaseAdoption] watcher faulting at the same instant and the OIB compile/release churn around it — so the key selector resolves to nothing for the in-flight snapshot. Predicts: transient — a hub initialization after the release settles succeeds. Reading: open the same node now; if it initializes cleanly, this cause fits and (1) is excluded.
  3. A lazy-initialization race in TypeDefinition: the key accessor being resolved lazily while five watchers initialize against the same type concurrently. Predicts: rare, never reproducible in isolation. Reading: whether TypeDefinition.GetKey (line 128 per the stack) dereferences a lazily-populated field, read from the framework source.

Readings 1 and 2 are cheap (one hub activation / one node read on memex-cloud) and between them decide data-defect vs release-window.
