Problem

After MAX_CONTINUATIONS (5), stop_reason: 'pause_turn' maps to 'other' with no tool calls (src/main/llm/anthropic.ts:223-250). The agent then records lastOutcome = 'answer' and stops (agent.ts:169-178) with no notice. A subagent would return the partial text as its answer.

Fix

Map an exhausted pause_turn to its own outcome (or max_tokens) and emit a notice such as "The model paused its turn too many times." Subagents should not treat that outcome as an answer.
