HonKit does not render with ebook-convert 4.x correctly.
This issue would be related to ebook-convert compatibility issue between 3.x and 4.x
The markup needed for header and footer templates when converting to PDF has changed. See the manual for details. https://calibre-ebook.com/new-in/thirteen
This change may be related.

Use ebook-convert 3.x.
