Form template structure revamp

Is your feature request related to a problem? Please describe. The present form structure doesn't look clean and has unnecessary column space to display the path route.

Describe the solution you'd like A much better structure where the template is divided into multiple sub-sections like (document selection and administrator path), ( delivery methods), ( purpose and other fields), etc in sheets kinda way wherein a user fills a subsection and can move to the next section through arrows in the bottom, similar to Google forms. Once submit is clicked, all the data entered should be made visible on the submit modal ( at present only path is visible).
