[Docs] Out-of-Cluster Meshery Server: Cluster Access and Connectivity Requirements

Current State

Meshery Server UI running out-of-cluster cannot access certain Kubernetes clusters if the cluster’s server URL (from kubeconfig) is not reachable from the server container.

Documentation does not clearly specify that the cluster server URL must be network-accessible to the Meshery Server for successful connections.

Users may experience connection failures when running kind, Minikube, or other clusters in separate containers or hosts.
