[FEATURE] QR Code generator form

Feature description: Create a form that asks user to enter type of data which to be converted to QR and then pop up name color etc details as mentioned

**Use case:**It will form basis for QR code generation.

Proposed solution: building a component in react envt styling using form.css something and then linking it to firebase if working on it by the admin to store data
