Research direction

Reproduce the GPT-5 Codex request with Copilot for Xcode 0.44.148 on Xcode 26.1 and macOS 26.1, then inspect the relevant files and logs under ~/Library/Logs/GitHubCopilot/. The issue has no reproduction steps or logs, so first establish the failing request details; done means the 403 behavior is explained and the requested behavior is verified in Copilot for Xcode.
