Follow the porting recipe in the parent issue. Blocked on the runner helper sub-issue.

Bindings, build integration and the request handler. Mostly one file per suite, so this splits well across several small PRs.

Fold public-api/startSpan-streamed back into public-api/startSpan once the helpers land.

Keep one suite from this group on the static pin as the guard for the package, and name it in the PR.
