Description

When using tab mode "buffers", creating multiple new files results in a single tab. Even :tabe only opens a new empty file in the current tab:

It's possible to switch between the buffers with :bn, but only a single tab named "No Name" is shown.

The unsaved buffers all have the name "No Name":

Investigate how the 'buffers' tab mode groups tabs. Look at the code handling tab creation when new files are opened, specifically around buffer naming (all unsaved buffers have 'No Name').
