Replace the existing `.github/issue template.md` with a new issue template following GitHub's latest workflow as described in the linked documentation (https://docs.github.com/en/communities/using templates to encourage useful issues and pull requests/configuring issue templates for your repository). The repository currently uses an old single file template; update to the new YAML based template format in `.github/ISSUE TEMPLATE/` directory.
